Care management and Wellbeing Reviews:

As professional Deputies or Attorneys, you have an obligation to frequently consider your client's wellness and determine if they may be entitled to free NHS Continuing Healthcare funding. We can help you meet the duties outlined in the OPG Professional Deputy Standards by conducting regular reviews of your clients' wellbeing.

All the above will be presented in a report, which will assist you to make further decisions on the work to be undertaken on behalf of the client and will help ensure you meet your obligations contained within the OPG Professional Deputy Standards (revised 2023).

 

We can also attend Care Act Assessments

This support will ensure clients receive the appropriate care and support tailored to their needs. A Care Act Assessment is an evaluation conducted by a local authority to determine an individual's care and support needs. This assessment identifies what assistance they may require to live independently and safely. The assessment is typically completed in a person's own home or in a care home and can also include input from  other professionals involved in the individual's care.

During these meetings, Continuing Healthcare (CHC) eligibility is also considered.

 

It is important for an advocate to attend Care Act assessments to ensure vulnrable client's needs are accurately represented and met. The goal is to develop a comprehensive and personalised care plan that addresses all aspects of the individual's wellbeing, from daily living activities to long-term health considerations. ​By attending these assessments, we ensure that clients' needs are fully understood and advocated for, leading to the best possible outcomes in their care and support plans.
